Required Medical Coder - Hyderabad

Key Responsibilities

1. Coding & Documentation

• Assign appropriate ICD-10, CPT, and HCPCS codes to diagnoses, procedures, and services.
• Review patient medical records, physician notes, and test results to ensure accurate coding.
• Translate complex medical terms into standardized codes for billing and insurance.
• Ensure coding reflects accurate severity, treatment, and resource usage .

2. Billing & Insurance Claims Support

• Prepare and process medical billing claims for insurance providers.
• Work with billing teams to ensure claims are submitted correctly and on time .
• Correct and resubmit denied or rejected claims after proper review.
• Minimize errors in claims to prevent financial losses and delays in reimbursement.

3. Compliance & Regulations

• Stay updated with coding guidelines, medical regulations, and insurance policies .
• Ensure compliance with HIPAA, CMS, and other healthcare data security standards .
• Follow industry best practices and company policies to maintain accurate documentation.

4. Quality Assurance & Audits

• Participate in internal audits of coded medical records.
• Identify errors, discrepancies, or incomplete documentation and correct them.
• Provide feedback to physicians and healthcare staff on documentation improvements.
• Maintain high coding accuracy rates in line with industry standards.

5. Communication & Coordination

• Collaborate with physicians, nurses, and administrative staff to clarify medical details.
• Respond to queries from insurance companies, billing departments, and patients when required.
• Support team members with training, updates, and knowledge sharing .

Required Skills & Competencies

Technical Skills:

• Strong knowledge of ICD-10, CPT, HCPCS coding systems .
• Familiarity with medical terminology, anatomy, and physiology .
• Proficiency in medical billing software and electronic health records (EHR/EMR) .
• Ability to analyze medical reports and assign appropriate codes .
• Awareness of healthcare compliance standards (HIPAA, CMS, etc.) .
