Key Responsibilities:

1. Medical Coding:

• Read and analyze patient medical records, operative reports, discharge summaries, pathology reports, and other clinical documentation.
• Assign appropriate diagnostic and procedural codes using ICD-10-CM, CPT, and HCPCS Level II coding systems.
• Ensure codes are accurately applied based on coding guidelines and payer requirements.

2. Claims Preparation:

• Translate medical information into numeric or alphanumeric codes to support insurance claims and reimbursement processes.
• Ensure accuracy and completeness in claim documents to minimize denials and rejections.
• Work with billing and revenue cycle teams to process claims quickly and efficiently.

3. Quality & Compliance:

• Adhere to all coding compliance guidelines from CMS (Centers for Medicare & Medicaid Services), AAPC, or AHIMA.
• Ensure medical codes meet legal and insurance rules as well as internal policies.
• Participate in audits and quality checks conducted by internal teams or external agencies.

4. Coordination & Documentation:

• Collaborate with doctors, nurses, or coders for clarification when documentation is incomplete or ambiguous.
• Maintain detailed logs of coded cases, productivity, and accuracy rates.
• Participate in team meetings and training sessions to stay updated with changes in coding regulations.

5. Training & Development (for freshers):

• Undergo coding training for specific specialties (e.g., radiology, surgery, cardiology, emergency services) if required.
• Complete certification training if not already certified (CPC, CCS, etc.)
