Requirement:& Opening in Medical Coding.&

Medical Coding is a process of converting Diagnosis, Treatment and Procedure into alpha numeric codes.& Eligibility:& BSc & MSc - Botany, Mathematics, Chemistry, Physics & Computer Sciences& immediate joiners preferred

Key Responsibilities:

1. Medical Record Review:

• Examine physician notes, operative reports, discharge summaries, lab reports, imaging results, and other clinical documents to extract relevant diagnoses and procedures.
• Ensure records are complete and support the coding requirements before assigning codes.

2. Code Assignment:

• Assign accurate ICD-10-CM codes for diagnoses, CPT / HCPCS codes for procedures and services performed.
• Use industry-standard coding manuals and software systems for referencing.
• Apply coding modifiers and ensure proper sequencing of codes based on payer guidelines.

3. Compliance and Accuracy:

• Follow coding guidelines established by CMS (Centers for Medicare & Medicaid Services) , AAPC , or AHIMA .
• Ensure coding is compliant with federal regulations, internal standards, and HIPAA laws.
• Maintain high levels of coding accuracy to avoid claim denials and audit risks.

4. Claims Support and Denial Management:

• Collaborate with billing teams to ensure coded data is ready for submission to insurance companies.
• Assist in correcting and re-coding denied claims or requests for additional documentation from insurance providers.
• Identify trends in claim rejections and recommend coding improvements.

5. Auditing and Quality Assurance:

• Participate in regular internal or external audits to verify coding accuracy and documentation compliance.
• Respond to queries from audit teams and implement feedback to improve practices.

6. Communication and Coordination:

• Interact with physicians, nurses, and healthcare professionals to clarify ambiguous documentation.
• Coordinate with billing, accounts, and RCM departments for claim-related queries or disputes.
• Provide feedback and guidance on documentation improvement when necessary.

7. Reporting and Documentation:

• Maintain accurate coding logs and productivity reports as per organizational policy.
• Generate coding error reports and contribute to process enhancements.

8. Continuing Education and Updates:

• Stay current with medical coding updates, changes in ICD and CPT codes, payer rules, and healthcare laws.
• Attend workshops, webinars, and certification updates as required.
